I understand that as an Associate Member of SEIU Local 2015 I may be eligible to participate in certain benefit programs of Local 2015, but I am not entitled to representation by SEIU Local 2015 with respect to terms and conditions of employment. I hereby certify that I am not employed in any bargaining unit for which SEIU Local 2015 is the recognized bargaining agent for matters relating to wages, hours and other terms and conditions of employment. I also hereby affirm that I am not an employer, or an agent of any employer, of any employee(s) represented by SEIU Local 2015.
