SEIU Local 2015 provides an array of options to our members for health insurance, education, legal protection, and retirement savings for you and your family. We’ve outlined them below and provided links to the enrollment portals online where applicable.

MetLife DHMO and MetLife DPPO Dental Plans

DHMO covers orthodontia, which is the treatment of irregularities in the teeth caused by alignment or occlusion. DPPO plan allows members the freedom to choose their own dentist. Both plans include implant coverage.

What is the difference between DHMO and DPPO Dental?

With DHMO Dental, you must choose a dentist that you are assigned to and is the gatekeeper for all of your care. Each covered service requires a set co-pay which can be found on the plan’s benefit summary. List of benefits summaries can be found here, in the right hand column.

With DPPO Dental, you are able to receive care at any dentist you choose. You will receive a better benefit if you see a dentist that is contracted and in-network with MetLive, but you also have the freedom to receive care from a non-contracted or out-of-network dentist. Your costs for services is based on a percentage of what the dentist is billing, rather than a set amount. There is also an annual maximum on your benefits which means MetLife will over cover benefits up to the annual maximum amount.

MetLife PPO Vision Plan

The PPO Vision Plan provides coverage for exams, lenses, contact lenses and frames. Additionally, there is a discount provided for LASIK procedures and a large provider network to better serve you. The plan also provides for an additional frame allowance.

MetLife Accident Insurance

The accident insurance provides a lump-sum benefit for injuries resulting from a covered accident. A benefit for AD&D is included as well. Covered accidents can include, but are not limited to, fractures, being admitted to the hospital, and visiting the emergency room.

Anthem Member Life/Accidental Death and Dismemberment (AD&D) Insurance

Under the AD&D plan, all members are approved for $30,000 with no medical questions or exam required. There is a double payout if the accident results in death.

Hyatt Legal Plan

The Hyatt Legal Plan allows for unlimited consultations. There are no deductibles or copays. The plan offers access to an attorney for a variety of legal needs such as immigration consultation, landlord/tenant negotiations, assistance with Medicare and Medi-Cal documents, and much more.
